Below is some important information for the summer and upcoming school year.  

IMPORTANT DATES

• June 27th & July 18th - sports physicals provided at Ascension - please see details below
• August 12 - Fall sports tryouts / practices begin for all fall sports
• August 15 – Fall sport parent meeting at 7 PM in the Heritage Vasher Theater
• August 20 – First day of school
• September 27 - Homecoming game and parade
• September 28 - Homecoming dance
• October 28 - Winter sports tryouts /practices begin - start dates vary by sport
• February 15 – Annual Hawktion athletic fundraiser – Please mark your calendars
• March 10 – Spring sports tryouts / practices begin for all spring sports

SUMMER CONDITIONING

FINAL FORMS

Heritage Athletics continues to partner with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.  

• Once registered you may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any changes/updates.  To begin or update go to :   https://saginaw-mi.finalforms.com
• All athletes are required to have a FinalForms account with a current physical uploaded.  Physicals must be loaded onto your student's FinalForms and dated on/or after April 15, 2024  to be eligible to participate. 
• A one time, non-refundable $100 Athletic Registration Fee is due from each student athlete. 
• Please find FinalForms directions and a blank physical form attached for your convenience. 

PHYSICALS

• What:  Sports physicals for the 2024-25 school year
• Who:  Grades 9-12 ONLY!  Incoming Freshman through Seniors.  NO EXCEPTIONS!  
• When:  Thursday, June 27 and Thursday, July 18
• Time:  4:00 PM - 7:00 PM (walk-ins only; no appointment needed)
• Where:  Ascension Medical 4677 Towne Center, 2nd Floor; Medical Arts #3 Building; Saginaw
• Cost:  $20 Cash only

What you need to do: 

• Take a completed MHSAA physical form with you to the physical.  The form is attached for your convenience (physical card new).  Students may also pick them up from athletics on open office days.
• A parent or guardian MUST complete the MHSAA physical form prior to the physical.
• Signature sections 1-4 MUST be completed by both guardian and student.
• Do not complete the "PHYSICAL EXAMINATION & CLEARANCE" SECTION - THIS IS FOR THE PHYSICIAN.
• Bring or wear prescription eye glasses or contacts if applicable for vision test.
• As a reminder, students MUST have a physical on file through FinalForms in order to participate in Heritage sports.  If your student does not yet have a FinalForms account please find the step by step directions attached for your convenience (parent-student playbook). 

ATHLETIC PASSES 2024-25 SEASON

ALL athletic passes (student, adult and family passes) will be available for purchase online this year.  Stay tuned for the link and date they become available.
