Heritage Students interested in participating in Heritage Athletics MUST have a Final Forms account with a proper physical uploaded in order to participate.

Physicals MUST be dated on or after April 15, 2023 in order to qualify.  All physicals MUST be uploaded to the student's Final Forms account. 

PLEASE DO NOT BRING THE PAPER COPY OF THE PHYSICAL TO THE SCHOOL.
