Heritage Athletics is excited to announce that Ascension Medical Group is offering sports physicals for the 2024-25 school year.  Please see the details listed below as well as the attached flyer.

• What:  Sports physicals for the 2024-25 school year
• Who:  Grades 9-12 ONLY!  Incoming Freshman through Seniors.  NO EXCEPTIONS!  
• When:  Thursday, June 27 and Thursday, July 18
• Time:  4:00 PM - 7:00 PM (walk-ins only; no appointment needed)
• Where:  Ascension Medical 4677 Towne Center, 2nd Floor; Medical Arts #3 Building; Saginaw
• Cost:  $20 Cash only

What you need to do: 

1. Take a completed MHSAA physical form with you to the physical.  The form is attached for your convenience (physical card new).  Students may also pick them up from athletics.
2. A parent or guardian MUST complete the MHSAA physical form prior to the physical.
3. Signature sections 1-4 MUST be completed by both guardian and student.
4. Do not complete the "PHYSICAL EXAMINATION & CLEARANCE" SECTION - THIS IS FOR THE PHYSICIAN.
5. Bring or wear prescription eye glasses or contacts if applicable for vision test.
6. As a reminder, students MUST have a physical on file through FinalForms in order to participate in Heritage sports.  If your student does not yet have a FinalForms account please find the step by step directions attached for your convenience (parent-student playbook).
